Bars/restaurants in new office buildings in Tokyo have nice clean restrooms that are in the building space. If you do not feel guilty (know that no other customers do not need to use) the handicap/baby changing restrooms are both large and very clean,late at night they are usually not in use. Hotels also are a good bet.
I'd really like to do it in the narita express on my way to narita lol
