Thank You - Sankyu - San(3)Kyu(9), the price 3,900 yen for 30 mins reflects the name, obviously the price is somewhat higher with nomination fee and some cast have special nomination fee but it is still dirt cheap. Their homepage lists a total 29 locations all over the country.
Apparently they have a questionable past but cant be bothered to dig in to that, I am sure some knowledgeable punter will add that in comments later.
The only times I ever got of at Uguisudani Station have been for walks to Yoshiwara but this time I use the north exit and randomly pick Hotel Station, to my surprise 5 drinks are included.
