I've seen the stag films and from what I understand, this wasn't unheard of in Japan. Given the huge interest in photography in Japan, I'm betting there's a lot of material buried somewhere...

Well, that was a bit of a joke, but it's definitely true that in the 80s or the pixel circle was so large on the screen that many if not most porn stars didn't actually have sex on camera. It's the rough equivalent of the dry humping that you see in American softcore porn.

Not really, no. The interesting thing is that MacArthur was the one that had the Japanese ban prostitution (for the sexual health of his soldiers).

Porn is censored because in many ways East Asian societies are just as sexually conservative as the US is, at least 'outwardly'. Prostitution is quasi-legal in a lot of Asian societies - It's easy to look the other way when a small section of town is a red light district (and that's considered an essential part of 'health). It's not easy to look the other way when a barely legal Asian woman is blowing 3 guys on camera. Porn is illegal to produce or sell in virtually every East Asian country. In Singapore, you can get in pretty deep shit just by bringing porn into the country. Japan is by leaps and bounds the most liberal of these countries when it comes to porn. Their brilliant solution to the 'blowing 3 guys in camera' problem is to use a mosaic, because then people can safely look the other way because after all, the woman really isn't blowing 3 guys, right?

Off topic but for the life of me, I will never understand the appeal of that kind of porn. I don't get the psychology of wanting to see so much cock on screen with only one woman.
 
I wonder if the censorship is part of the reason why there is so much female moaning in Japanese porn. Perhaps the moans help to make up for the limited visuals?

IRL some women do moan, but the moans often seems exaggerated in JAV.

I don't know, in American porn, the women always shout stuff like "fuck yeah" very loud and do a lot of dirty talk and over the top moaning as well, which is also not realistic.
