I would definitely wait till you get back to Tokyo. The overall accessibility and quality of massages is in another league. Osaka's only real claim to P4P fame is Tobita Shinchi, and while it's cheap and fast and can often be a blast, you can also score some real duds there if you go often enough.
Regarding Spa World, it's truly the only thing I still enjoy in Osaka. I would say it's similar to Oedo Onsen Monogatari except it's sigificantly cheaper (Spa World lowered it's daily pass from 3,000 yen to 1,200 yen some years ago and kept it there), and Spa World is much larger. It's a massive complex where the 4th floor is the Europe floor and the 6th floor is the Asia floor (or maybe the other way around?), and they alternate between sexes each month. It also has a pool on the top floor, which means you (unfortunately) get a LOT more kids in there, running around, screaming, horseplaying, etc. I would say, if you go to Spa World, go on a weekday. Assuming you're a straight male, I would also avoid it late at night. I've been there a couple of times just after arriving, still drunk from the flight, and judging by the glances and gawks I got from a lot of the men lounging around, I got the distinct impression that a lot of homosexuals hang out there at night for the sole purpose of scoring a hook-up. Kinda like the YMCA in the 1970s, I suppose. And they have these really interesting communal steam rooms where you can barely see three feet in front of you--I imagine such rooms could come in handy in that situation. Not judging, as if I were of that persuasion I'd probably be there six nights a week. Just something to keep in mind.
But screaming kids and leering queers aside, I still love Spa World. In Tokyo, Oedo is a little too much like a theme park for me, with crappy overpriced food and drinks, and at the price they charge I prefer Spa Laqua. But Spa World has a fabulous rotenburo area, and right next to the in-house bar! Grab a couple Asahi drafts with some edamame, get a good buzz going, walk out and sit in the boiling water, repeat, repeat, repeat.
On a side note, if you're on a budget and like drunken bathing, DO stop at a convenience store to grab some Asahi tall boys and bring them in your backpack. Spa World draft beers are something like 700 yen now, and they got rid of the overpriced but still cheaper beer vending machines on 3F. But if you're doing the 60 minute akasuri course I'd suggest you lay off the drinking till after your course is finished, unless the prospect of making a special yellow present on the wash table for the nice Korean lady is something that tickles your fancy.
Hell, I'll have to try hard to follow that advice. I plan to hit Spa World in about two weeks.
