I'm a male in my 20's and straight.

I decided to take a trip to Japan and everything was great sightseeing and eating and eating more. Last night of the trip before flying back home, I ended up going to a love hotel with an escort. It was one of the escorts specializing in anal. I used one of those 0.001mm condoms they sold in Japan but broke during sex. I didn't notice until after I realized I ejaculated in her and pulled out (Probably lasted about 10 min broken).

It's been about 2 weeks since that happened and I am so paranoid about if I am infected with HIV. I read that there is a higher risk of getting infected through anal. I looked up the symptoms and do not have any flu like symptoms or any rashes or swollen areas. The part that scares me is that the joint on my left thumb started to hurt a little bit when I pressed somewhat on it. I read that joint pain is also one of the earlier symptoms. No neck joint pain or thigh area pain as of now. I did have skin peeling on my arm and face but that was due to getting sunburned and looking like a tomato. I have seen a few pinkish dots on my arm and stomach area but I think that's more towards a pimple since I always had skin problems on my chest and back (nowhere near the redness and density of a typical rash). I am waiting for one or two more weeks to get a 4th generation test since I heard that is the earliest that test can detect or maybe the in-home kits.

Is there a high chance I did get infected or is my paranoia psychologically making it a lot worse than it seems?

This is the logical answer.

Go get tested if you want, but go to a local clinic and follow their guidelines for testing.

Just like others in the past, you will dwell on this and make yourself believe you have something when you really don’t.

The receiver in anal sex has the highest risk of transmission of all sex acts, (I’m not including non sexual procedures like blood transfusions) but you would do the right thing with getting tested. The escort is in a more vulnerable position at the moment.

Almost everyone will have pinkish dots on their stomach and arms at some point.

I purchased those 0.01 and 0.03 condoms and compared them to my other Okamoto’s. They are prone to breakage and the 0.01 just broke after some pushing and pulling. Recently Okamoto has come up with a new lube to use with the thin condoms but water based lubes still dry up and I am pretty strict about not using those with clients.

It's almost certainly paranoia. If an HIV-negative top—the insertive partner—and an HIV-positive bottom have unprotected sex, the chances of the top contracting the virus from a single encounter are 1 in 909 (or 0.11 percent) if he’s circumcised and 1 in 161 (or 0.62 percent) if he’s uncircumcised.

I get that the paranoia has set in, but the odds are EXTREMELY small.
For you to have contracted the virus, the escort you were with would have to have it in the first place. That in itself is a very small chance. And then to contract it from her?
You're doing the right thing getting checked out, but in the meantime, don't overstress yourself. You're probably doing more harm to yourself by worrying so much.
 
I had a similar problem years ago. I had unprotected sex with my ex twice after she broke up with her boyfriend and during that time from our break up to us having sex again she been been with at least 2 other guys that I know and she didn't make them wear condoms either. At first I was paranoid and then a year later when I was living abroad I was getting bad diarrhia from the water at first and was super paranoid that it was some how HIV perhaps. I don't suppose this escort was from an agency with a website that has her last check up? If she was cleared relatively not that long ago that could help with the stuff of waiting.
 
I remember reading statistics online the risk of males getting HIV was easily under 3% going bare. I've had a few of these scares similar to yours and my fear was the biggest threat after getting tested each time. Get tested 3 months later.
 
Planning on getting tested with the HIV RNA test this week which can detect infection before the 1st-3rd month of possible contact with HIV. A little more worried though because I got a little nausea and felt my head get a little hot for about an hour but no more nausea after that. I heard that this symptom happens for a long longer so I'm leaning towards this probably from stress and getting 4 to 5 hours of sleep everyday from anxiety.

In all honesty, none of us want to hear a blow-by-blow of your anxiety. Talk to a doc, a shrink, or a priest of that's what floats your boat.

It’s normal to have anxiety during STD testing if you’re new into P4P or if you had an “accident” like this.. and anxiety gives you nausea.

We have posted lots of info above to help you feel better, and you can make your own judgement upon that.

If you ask me, even if the provider has HIV which is a minimal risk in Japan, escorts get tested religiously & will know their status. HIV positive people who are diagnosed usually use pills that will drop the chances of transmitting the virus to a partner to virtually zero.

If I were you, I would take this as a lesson and use thicker condoms in the future for riskier sex acts like anal. I know it’s not worth telling someone with anxiety to stop worrying but hope this helps.

You are more likely to catch HIV from multiple intercourses with a random person who doesn’t go to health screenings.
 
get some anti anxiety meds, you probably a better chance of winning the powerball.
 
We all know the chance of HIV infection is extremely low, but after an event that has triggered anxiety, paranoia sets in because you face a binary outcome. It's difficult to overcome. It was difficult for me when I was in the same situation as the OP (I didn't do anal tho). I would recommend the OP to go to a clinic and talk to a doctor even if it is too early for a test. You may need to vent, but it's better to do it to a pro.
 
Don’t worry! I’m super paranoid and I’ve read ALL the forums, statistically speaking your risk is negligible. The RNA test is conclusive after 2 weeks, the 4th gen is 95% or higher at the 1 month mark. I would be more worried about other STDs, that luckily under normal circumstances are treatable or you can pretty much ignore.
 
As a guy, the odds of getting HIV in Japan from a single instance of unprotected sex from vaginal intercourse is basically a one in a million chance when you take into account both the extremely low prevalence of HIV in Japan as well as the extremely low chance of contracting HIV from penetrative sex. You should be much more concerned about (entirely curable, but still nasty) STIs like Gonorrhea, Chlamydia, or Syphilis, which are both much more common and much easier to catch, although the odds of catching any of those is still pretty tiny. All STIs are either treatable or curable, so like everyone else has mentioned go get tested, but until then there's no need to panic.

I’d still say its small chances because very few Japanese people have HIV and the giver of anal is in less risk but anal is the more risky activity.

Bareback anal is riskier, but obviously much more riskier for the recipient, especially if you cum inside.
