Back on the "System" page, click on "Reserve" at the top to open the online reservation form: http://www.milkybaby.jp/reserve.html . I completed the form using only English. For "Desired course", I chose the "breast milk course 120-minutes" with a "chikiban" (mutual nipple-licking) "option". I also registered for the Email magazine (http://www.milkybaby.jp/melmaga.html) because a code is sent to your email address which speeds up your booking if you enter the code on the online reservation form. Next to "The woman you want" I requested "Omakase" (they choose), and for "Love Hotel" I chose "Other". Then, in the "Remarks" box, I wrote a short description of myself and what I wanted, asked them to choose the love hotel, and added a school girl costume request. (They have lots of cosplay options: http://www.milk-club.net/op/costume.html). And hit "send"!
About 1-hour later, I received an email from Milky Baby (in English and reproduced in Japanese), thanking me for my reservation, suggesting two girls, and requesting I phone the shop a total of 3 times: the day before the booking, when I arrived at Ikebukero Station, and when I was outside their building. I replied to the email, choosing Ren - quoted at 42,000¥ for 2-hours + 2,000¥ for the school girl costume, (the chikiban option was free, as was registration with the shop), totalling 44,000¥ - and confirming the phone instructions.
The day before the booking, I phoned as instructed and spoke to a very friendly Mama-san who had some basic English skills and she confirmed my booking with a cheery "See you tomorrow!"
The website has a helpful map to the shop, http://www.milkybaby.jp/map.html , which is in a ten storey office building(?) or mansion tower(?) about 10-minutes walk from Ikebukero Station East Exit (North). Here I made my second phone call to confirm I would be there shortly. Once outside the building, I made my third phone call and was given the floor and room number. Once outside the door, I rang the bell, and a friendly, middle-aged lady let me in, instructed me to take off my shoes, and showed me to a seat beside a desk. Behind the desk was another room with a group of women (presumably on duty today) sitting and chatting.
I was first issued with a Membership Card with a number which the Mama-san recommended I entered into the contacts card for Milky Baby on my iPhone. I dutifulky did so. I was then asked for my passport and a younger woman from the back room came through to verify my birthdate and identity from my passport. Mama-san showed me a booklet with Ren's page open and her face visible. Once I acknowledged that I liked what I saw, Mama-san asked for the payment of 44,000¥.
Ren then came through from the back room. We made our introductions and left the office, chatting all the way to a nearby love hotel, partly in English and partly in Japanese.
Inside the hotel (Clio), Ren said to the person behind the window, "Milky Baby", and I was asked to pay 3,700¥ for the (pre-chosen) room for 2-hours. We went to the room and settled in.
