I did a big mistake last night (vaginal sex with a without protection)...
I heard about the day after HIV protection pills that can greatly decrease the risk...does anyone of you knows how to get it in Tokyo?

To be emphasized it is used only when you know your partner has HIV. The side effects of the drug can be really bad and therefore they do require you to be monitored carefully for at least a week.

Because even if your partner had HIV it is very unlikely you managed to catch it unless you shared a needle.
 
Thank you for your feedback. I understand this may not be the right way to do for me at the moment. For sure we did not share needle, and I dont know if my partner had HIV but still vaginal intercourse may put me at risk..anyway you think PrEP is not the right solution ?

I’d be more concerned about chlamydia etc rather than HIV to be honest. As the others have already said, it’s less likely you catch HIV compared to other infections. Get tested in maybe 1-2 weeks and make sure to cover chlamydia, gonorrhea, Syphilis and the like.

is the escort from a shop? or is the escort one of those tourist ones? (get in Japan via short term tourist visa and work from their hotels)

escort from shop requires monthly checkups ... it is possible for an infection to occur but chances are low. not zero, but low. so i would suggest not playing (having raw sex, or other play that transfers bodily fluids) for 2months (incubation period) and then having an STD panel performed (they draw blood for testing).

tourist by day/escort by night can be more problematic. since no one is there requiring them to have monthly checks.

For many reasons, you should go see a doctor. Dont be shy, explain what happened, theyve heard it all 100 times before.

The chances of this provider having AIDS are close to 0, the chances for a man of contracting AIDS from vaginal intercourse from a partner are quite low. The chances of contacting AIDS form one single intercourse are close to 0.

But, as others said, check for chlamydia, gonorrhea before playing with someone else.

As others have said already, the risk of HIV is extremely remote. Even in Thailand where it used to be an issue, the routine use of condoms has pretty much eliminated that risk.

Yes, the risk is not zero but there are other concerns that are more realistic (other STDs). But those are manageable with proper treatment and medication. Calm down, refrain from sexual activity for a few weeks and then get yourself tested. Let the doc know what happened and let him take it from there.

a simple white lie might also keep you *low key*

i've told one doctor that i managed to pickup a girl for an ONS ... while doing the deed the condom broke and went raw ... the first part is the lie that kept me from mentioning *escorts* to my doctor

The normal answer to that is I didn't think it through beforehand and I got carried away.

Which is a good lesson to any young, and some old, guys here: always think how you are going to behave in different situations before you get in to those situations.
 
I just see I was drunk beyond belief a common reason with me & friends. So I’m asking. Time with plastic wrap on the pecker lots of alcohol was involved.
 
Hello, thanks again to you all. To answer the question, yes I was drunk, and it was a sumata that turned to a penetration. It did not last long though when I was realized I was inside. Still long enough to scare me...

Something nobody mentioned yet is the timeline of when tests will actually be able to show whether you have anything.

Almost all STD's require at most three weeks for their presence in your blood to be of a sufficient level that a test will be conclusive. So three weeks from the deed (or sooner if visible, unmistakable, symptoms appear) go for your first test.

HIV however requires at least three months. And you will need to test again three months later to make sure it just wasn't a slow start.

You`ll notice that even if she was HIV+ you are still in the "little or no risk" stage.

In fact, assuming the worst case, where the escort has acute HIV+ (which is when the viral payload is the highest) and suffers from others STD`s (which makes it easier for HIV to be transmitted), you still wouldn`t be in the high risk category. Note; it`s also also very unlikely you would hit those worst case assumptions.
 
Honestly... if someone offers you raw sex, you might not be the only one she’s making an exception for. I don’t know what kind of massage place this is, but I’m assuming if BJ or FS is not on the menu STD tests will not be mandatory for the girls? (I might be wrong) Statistically your risk of contracting HIV is extremely low, but please listen to others and get a comprehensive STD test for bacterial infections.
 
Even though you had unprotected sex with a Thai prostitute, your chances of having contracted HIV are lower than you already having some other undiagnosed incurable fatal disease that will kill you in the next two years. Brain tumor, pancreatic cancer, etc. You could always also get hit by a truck or get stung by a killer bee. Feel better?
 
What you want for HIV risk mitigation after the fact is PEP, PrEP is for beforehand.

The advice here is correct though, the risk factor for HIV for you (a single vaginal encounter with an unknown-status partner) has a lot of zeros in the figure. Likelyhood of other STDs is much higher relatively speaking.

(Generic PrEP is quite affordable these days though, on the order of 3000 yen/mo shipped to Japan but the most reliable source I know of will only ship a 1 month supply to Japan at a time.)
