Check out fetlife and search for Osaka’s S&M and Fetish group, they organise “munches” which is a meet up at a bar or restaurant and you should be able to meet some potential hook ups

Unfortunately, most folks here are more plugged in to the Tokyo scene... if you ever decide to travel here, we'll be a LOT more helpful.

I recommend to head to Doyamacho then and check out some of the bars around there. Global kiss and explosion are good.

I concur with the above poster. Doyamacho, which is near Umeda/Osaka station, has emerged as Osaka's most famous LGBTQ neighborhood and there are reportedly a number of clubs there frequented by the trans/gay crowd and those hoping to get lucky with one of them.

If you're familiar with Osaka Station, head toward Hep 5 and then past Don Quijote and keep walking straight. You'll know when you're there.
