I think you should read the whole sentence without the coma after that part. The whole meaning if I understood well is that people from poor countries do it because of lack of other opportunities, whereas in rich countries there is not such « excuse ».
 
I'm not exactly fine with it because I personally choose to follow the rules visa-wise, but I think it's a little hypocritical to be perfectly fine with prostitution (breaking the law) while in the same breath telling people to not break the rules. The fact that some people want to 'live beyond their expenses', in other words, get ahead in life for themselves or their family, is the reason prostitution can exist in the first place. Women certainly don't do it for their health and generally don't do it for fun.
