Japanese couples generally engage in oral sex only as foreplay, even as an escort I have never had a Japanese guy even ask or try to CIM; I think from a Japanese perspective a BJ is more like something you do to get warmed up for regular sex. Aside from the taste/smell which a lot of women are averse to, many don't like CIM because it's a lot of work, with no pleasure on their end (how are you gonna put your dick in them after you already blew your load?). It makes a huge difference. Guys can smell down there without realizing it even if they look clean and generally have good hygiene, and sometimes that smell can be really unbearable but 99% of the time it's something that can be taken care of with a quick shower.

The disease risks from BJ are generally associated with the throat (or basically anywhere with mucous membranes). Swallowing will definitely bring it into contact with the throat, but not swallowing is also risky as it will likely hit the upper area of the throat anyway.

As I'm sure you are well aware, there are various solutions to this "problem", foremost among them being that the guy can return the favor and get her off orally...perhaps accompanied by penetration with fingers, a dildo or whatever. Indeed, there are lots of women who can't come without good clitoral stimulation and who find receiving oral the best option. Other solutions are that many guys, especially younger ones, can get hard a second time after a short break in the action and of course the couple can do vaginal intercourse for the first round and then go to oral. There are also guys who come too quickly the first time (especially) to finish satisfying their partner with intercourse but can do so if they have already cum once from a BJ.

Anyway, oral sex can be great in love making in a nearly endless number of ways.

@Wwanderer that is true, some guys are able to go for more than one round relatively quickly. Ultimately it's up to how well they know their own bodies and their partners and how they communicate those needs. There are some people who are, no matter what, not going to be down for certain things, others might be more open minded if they could be assured of reciprocation.

A shot of (good quality) brandy is rarely a bad idea! :D And it probably does reduce the (already very small) chances of getting an STD somewhat.

However, not everyone finds either the taste or the texture unpleasant. I've had my own cum in my mouth, and while the taste was not particularly yummy imo, it also wasn't particularly bad. The texture is indeed quite slimy, but that bothers some people not at all. Personally I enjoy eating some foods which are at least as slimy in texture...in fact I can't recall ever having any food in which the texture bothered me significantly. And, of course, there are women who seem to enjoy playing with cum in their mouths...
