Well it depends. In bourgeois cercles or when the person is not well known, yes a real kiss is not done, rather a mimic of a kiss. But among friends, in informal situations or in relaxed circumstances, a real kiss is made on the cheeks.

Concerning the rest of your interrogations, no offense but you are maybe misunderstanding what la bise is for. You should not sexualize it or view it as part of sexuality. La bise is a way to socialize and create contact with people in the same way saying "bonjour" and smiling to someone are the basics of politeness. La bise is the part of saying "tu" to someone, it has fraternal and egalitarian values into it.
